A regional healthcare organization in England is seeking a motivated Nurse Assessor to join their Learning Disability Team. This role involves conducting assessments for vulnerable individuals, managing caseloads, and promoting high-quality patient care and personalization initiatives. As a vital member of our Learning Disability (LD) Team, you will have a unique opportunity to make a difference in the lives of vulnerable individuals within our community. Your role as a Nurse Assessor will be instrumental in ensuring the delivery of high-quality care and contributing to cultural change to achieve outstanding results.
This is an exceptional chance for a motivated and experienced registered nurse, ideally with an RNLD qualification or substantial LD field experience. You will use your assessment skills to support and advocate for some of the most vulnerable individuals, contributing to the assessment and review processes for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC), including the promotion and evaluation of Personal Health Budgets.
