Central London Community Healthcare NHS Trust is seeking an experienced Learning Disabilities Case Manager to join the CHC team in North West London. An exciting opportunity has arisen for an experienced and motivated Learning Disabilities Case Manager to join the Continuing Healthcare (CHC) Team, supporting individuals with learning disabilities across North West London. This is a rewarding and influential role within a specialist service dedicated to improving the lives of people with learning disabilities and complex health needs. You will play a key role in ensuring individuals receive personalised, high-quality care that promotes independence, choice, dignity and the best possible health outcomes. Working at the interface of health, social care and commissioning services, you will coordinate complex care packages, undertake assessments, advocate for individuals and their families, and work collaboratively with a wide range of professionals to ensure care is delivered safely, effectively, and in line with NHS Continuing Healthcare guidance. This role offers an excellent opportunity for an experienced Registered Learning Disabilities Nurse (RNLD) or healthcare professional who is passionate about reducing health inequalities, improving experiences of care, and influencing service development for people with learning disabilities.
