Job summary

Lancashire & South Cumbria NHS Foundation Trust is recruiting a Band 5 Learning Disability Nurse to work within the Specialist Community Learning Disability Team. You will support adults with learning disabilities with complex needs in community settings, alongside families and multi-disciplinary colleagues.

The role involves clinical activity, delegated duties from senior staff, and contributing to high-quality care within integrated health pathways.

As an integrated Trust, Lancashire and South Cumbria NHS Foundation Trust (LSCFT) delivers a comprehensive range of services, including:

• Primary and secondary mental health care for children and adults, including specialist inpatient child and adolescent mental health provision, perinatal mental health, and forensic services with low and medium secure care.
• Specialist mental health inpatient care for individuals with learning disabilities, alongside specialist community support for children and adults with learning disabilities and autism, including intensive support.
• Extensive community physical health and well-being services for children and adults, covering prevention, treatment, rehabilitation, and long-term condition management.

The Trust was first established in 2002 and employs approximately 7,500 staff who provide care from more than 400 sites. The organisation offers opportunities for medics, mental health, general nurses, children’s nurses, allied health professionals, psychology, administration and clerical staff, apprentices and volunteers, as well as those specialising in learning disabilities and community physical health services.

An opportunity has arisen with the service, for a Band 5 Learning Disability Nurse post to work within the Specialist Community Learning Disability Team and innovative. The Learning Disability Service strives to provide forward thinking high quality services, working in partnership with other agencies. In addition to working with adults with learning disabilities and autism, we work closely with their families, and we have a strong commitment to health promotion through service development and training initiatives. We are committed to helping our staff develop and you will be part of an enthusiastic and experienced team.

The post holder will work within an integrated health team, providing clinical support to adults with learning disabilities who have additional complex needs, which may include challenging behaviour, physical and sensory impairment, mental health and forensic issues. The post holder will play an integral role in delivery of care within these pathways within a variety of community settings.

Main duties of the job

As a band 5 community nurse you will be expected to support the clinical activity of the teams in meeting the needs of local people with learning disabilities in the community. You will have skills in working with adults with learning disabilities, and associated communication and behavioural needs. The post holder will undertake delegated duties by senior nurses, and will be accountable to the Operational Manager and Senior Community Nurse and Locality Lead Nurse, and will be providing administrative support within the integrated health team, to ensure effective and timely clinical outcomes are met.
