Responsibilities

• Lead and develop palliative care services across community and hospital settings.
• Support rapid development of a 7-day rapid response service and Hospital at Home initiatives.
• Collaborate with senior clinical practitioners, specialist nurses, and other specialists to deliver integrated care.
• Provide leadership in service development to improve palliative and end-of-life care for Bolton residents.
• Contribute to on-call arrangements and pilot weekend working for the new service.
