Job summary

Lancashire & South Cumbria NHS Foundation Trust's Guild Lodge is seeking a qualified Social Worker to join Secure Services as a Forensic Discharge Pathway Facilitator (Inpatient Social Worker). You will contribute to holistic assessments, lead discharge planning, and work with a wide MDT to remove barriers to timely discharges.

Qualifications

• Must be a qualified Social Worker with Social Work England registration.
• Experience in secure mental health or offender health settings is desirable.
• Ability to lead on discharge planning and coordinate with MDTs.

Responsibilities

• Lead holistic social work assessments from pre-admission.
• Identify social work needs and barriers to discharge.
• Liaise with Local Authorities and other agencies for capacity and funding.
• Attend MDT, Care Team, MAPPA and Best Interests meetings.
• Support discharge to community with appropriate plans.
