Job summary

Salford Royal Hospital is seeking an experienced Social Worker to join the Neuro and Stroke Social Work Discharge Team. You will operate in a fast-paced acute hospital setting to support patients in returning to their communities and living independently.

You will work with a MDT to deliver personalised assessments and ensure timely, safe discharges. The role requires strong assessment, safeguarding, communication, and partnership-working skills, with a commitment to person-centred care and

Qualifications

Provide a person centred, statutory social work service for the most vulnerable adults and their carers in accordance with local and national policies and procedures.
Actively support and participate in the development and implementation of integrated, multi-disciplinary working.
Take a key role working with partners to safeguard service users through the effective management of risk and the recording and sharing of information.
Adopt a strength’s-based approach to meet the individual needs of people with lived experience through effective support planning.
Contribute to the effective development of the team.
Work across various wards and clinics, including specialist clinics (e.g., MND) across Greater Manchester.
