Job summary

South West London and St George's Mental Health NHS Trust is seeking a social worker to join a forensic community team (FIND) serving adults with LD/ASD and co‑morbid mental health needs across London boroughs. The role emphasizes care coordination, discharge planning, and MDT collaboration.

You will work autonomously within a multidisciplinary team, providing personalised pathways in line with recovery goals, with opportunities for development and flexible working arrangements.

Qualifications

• Post qualifying experience as a social worker.
• Experience of working autonomously within a multi‑disciplinary environment.
• Experience of working with people with complex mental health needs.
• Experience of working with people with LD and ASD needs.
• Ongoing evidence of continuing professional development.

Responsibilities

• Provide care co-ordination and social supervision for an agreed caseload.
• Prioritise discharge pathways to ensure timely and safe discharge.
• Offer consultation and guidance to colleagues and service users in the community.
• Develop and monitor care pathways and quality improvement plans with the MDT.
• Liaise with Community Care Coordinators to facilitate transfer from out of borough services.
