South West London and St Georges Mental Health NHS Trust is recruiting a part-time social worker for the forensic LD/ASD pathway. You will support adults leaving secure services across the SLP, coordinating care, providing social supervision, and reducing risks in discharge planning.

Qualifications

• Post qualifying experience as a social worker
• Experience of working autonomously within a multi-disciplinary environment
• Experience of working with people with complex mental health needs
• Experience of working with people with LD and ASD needs
• Ongoing evidence of continuing professional development
• Experience of working with service users in forensic services
• Experience of working with adults with substance misuse or dual diagnosis
• Demonstrate experience or skills to develop a positive leadership for social work profession in a MDT setting, as well as across wider Trust

Responsibilities

• Maintain a clear understanding of patients with LD/ASD in secure and specialist services across the SLP via CPAs and discharge management
• Monitor and review patients in medium/low security and out of area with timely, personalised pathways
• Carry out comprehensive social care needs assessments with key stakeholders
• Develop and monitor step-down pathways with personalised care packages
• Collaborate with Community Care Coordinators to transfer from out-of-borough services
• Hold a caseload as part of care-coordinator/social supervision duties
• Demonstrate autonomous, senior practice within a MDT with a focus on recovery and outcomes
• Develop policies and procedures with the MDT and ensure service systems are embedded and sustainable
