Job summary

Stonewood Healthcare Gateshead, UK is seeking a Registered Complex Care Manager to lead the mobilisation and day-to-day delivery of complex care packages, coordinating with families, commissioners and multidisciplinary teams.

You will recruit, train and supervise small care teams; embed policies, care plans and risk assessments; and ensure compliance with the Health and Social Care Act 2008 and CQC standards, safeguarding both children and adults.

Qualifications

• Experience managing or deputy managing domiciliary/complex care services.
• Strong knowledge of CQC regulations and single assessment framework.
• Experience with safeguarding for children and adults.

Responsibilities

• Lead the mobilisation and day-to-day delivery of complex care packages with families, commissioners and multidisciplinary teams.
• Recruit, train, supervise and retain a dedicated care team around the individual client.
• Maintain and embed policies, care plans and risk assessments meeting CQC standards.
• Ensure compliance with Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 and CQC frameworks.
• Oversee clinical governance, medication management, safeguarding and incident reporting.
• Manage rosters, care delivery quality and continuity of care with a hands-on approach.
• Build relationships with service users, families, ICB commissioners and health professionals.
• As the service matures, onboard additional specialist care packages.
