Job summary

Jarrodean in England is seeking a dedicated Care Coordinator to support complex care clients in home and community settings. You will work under the Registered Manager to ensure safe, effective, and person-centred care across the service.

The role requires coordinating care plans, liaising with clinicians and families, and covering on-call duties when needed. A proactive, flexible approach and the ability to work without supervision are essential.

Qualifications

• Minimum 12 months experience as a Care Coordinator.
• Knowledge of CQC regulations and Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
• Flexible, adaptable, confident to work without supervision and calm under pressure.

Responsibilities

• Deliver and monitor the delivery of complex care services.
• Provide day-to-day operational leadership across the service, ensuring safe, effective, responsive, caring and well-led care.
• Respond to short-notice staffing changes.
• Liaise with clinicians, commissioners, families, MDTs and other stakeholders.
• Act as deputy in the absence of the Registered Manager.
• Take part in the on-call rota.

Reporting to the Registered Manager, you will;

Person requirements:

• At least 12 months Care Coordinator experience
• A knowledge of CQC regulations, and the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014
• Personal qualities to include; flexible, adaptable, confident to work without supervision and calm under pressure.
• A current driving licence and access to a vehicle
• A commitment to a flexible approach to meet the needs of the service.
