Job summary

Islington Council in London is seeking a compassionate Community Support Practitioner Team Manager to lead our new preventative, person-centred service delivered in partnership with Enabling Change and Access Islington.

You will build trusted relationships with residents facing complex needs, coordinate multi-agency support, and help people move toward greater independence while maintaining accurate records and managing risk.

Qualifications

• Experience supporting adults with complex or multiple needs.
• Knowledge of mental health, homelessness, substance misuse, or social exclusion.
• Strong communication and relationship-building skills.
• Experience working collaboratively with partner agencies and services.
• Ability to manage a caseload and maintain accurate records.
