Job summary

Islington Council is recruiting a Community Support Practitioner to work in a new service delivered in partnership with Enabling Change and Access Islington. You will support residents with complex needs through a person-centred, preventative approach, helping them access the right help at the right time.

Qualifications

• Experience supporting adults with complex or multiple needs.
• Knowledge of mental health, homelessness, substance misuse or social exclusion.
• Strong communication and relationship-building skills.
• Experience working with partner agencies and services.
• Ability to manage a caseload and maintain records with accuracy.

Responsibilities

• Build trusted relationships with residents facing complex needs and provide tailored support.
• Coordinate support with colleagues, council services and community partners; address barriers and manage risk.
• Help residents move toward greater independence and wellbeing.
