Job summary

Islington Council, in partnership with Enabling Change, is seeking a compassionate Community Support Practitioner to help residents facing complex challenges build brighter futures. You will form trusted relationships, coordinate support across council services and partners, manage risk, and help people move toward greater independence with a person-centred, preventative approach.

Qualifications

• Experience supporting adults with complex or multiple needs.
• Knowledge of issues such as mental health, homelessness, substance misuse, or social exclusion.
• Strong communication and relationship‑building skills.
• Experience working collaboratively with partner agencies and services.
• Ability to manage a caseload and maintain accurate records.
• A strengths‑based, person‑centred approach and sound professional judgement.
