Job summary

Hertfordshire Community NHS Trust is seeking a Band 6 Community District Nurse Team Lead to supervise and develop a nursing team delivering community-based care by 8am to 10pm, seven days a week. You will manage a defined caseload, conduct care planning, and support a multidisciplinary team in admission prevention.

Applicants must hold an NMC registration as a Registered Nurse, a district nursing qualification and be willing to undertake further training including nurse prescribing.

Qualifications

• NMC registration as a Registered Nurse.
• Experience as a Band 5 community nurse or equivalent.
• Experience of managing a team and teaching others.
• Willingness to undertake training in district nursing and prescribing.

Responsibilities

• Lead and supervise the nursing team, setting standards of care and deploying, co-ordinating and supervising nurses and learners.
• Assess, plan, implement and evaluate individualised nursing care for patients living in the community.
• Hold continued responsibility for a defined caseload.
• Support a multidisciplinary team delivering clinical health and social care in admission prevention and avoidance.
• Take part in the long-term management of patients in the locality.
• Keep accurate patient and other records.

Skills

Mentor/Preceptor/998 or equivalent

Education

Specialist nursing practice (district nursing) degree, or willingness to undertake training

Nurse prescribing, or willingness to undertake training
