Job summary

Provide CIC is seeking an experienced district nurse for a Band 6 role based in Braintree. The post holder will work with the Community Matron to plan, allocate and review patient care, manage caseloads and develop care plans across primary care settings and social services, ensuring high-quality community care.

The role requires current NMC registration, ongoing CPD and a nursing degree, with possible Nurse Prescriber status.

Qualifications

• Current NMC registration
• Evidence of ongoing continuing professional development
• Relevant nursing degree or equivalent community qualification
• Qualified Nurse Prescriber or working towards

Responsibilities

• Caseload management and allocation of visit frequency.
• Formulation and review of care plans.
• Evaluation of care including first visits.
• Promote health by educating patients and carers per Health Promotion ethos.
• Liaise with primary care team, social services and other agencies.
• Develop and coordinate care and complex discharges, including palliative care.
