Job summary

Leicestershire Partnership NHS Trust is seeking a Band 6 Practitioner to join the Learning Disability Crisis Response and Intensive Support Team (LD CRIST) in Leicester. You will manage your own caseload within a multidisciplinary team, delivering clinical assessments, risk management and bespoke interventions to prevent inpatient admissions where possible.

The role supports people with learning disabilities and complex needs across Leicester, Leicestershire and Rutland, with rotating weekend

Qualifications

• Registered professional with relevant NHS Band 6 role.
• Full UK driving licence and access to a vehicle insured for business use.

Responsibilities

• Triaging new referrals from multiple sources.
• Provide clinical assessment, planning and oversight for patients in the community at risk of admission or placement breakdown.
• Offer training and advice to carers and health and social care staff on meeting individuals’ needs and Positive Behavioural Support plans.
• Provide specialist advice to enable community living and avoid inpatient admission where possible.
• Monitor pathways and handover to LD Community teams when required.
