Job summary

The CAMHS Crisis Team at Leicestershire Partnership NHS Trust provides specialist mental health crisis support for children, young people and families across Leicester, Leicestershire and Rutland. As Lead Professional for home treatment, you will coordinate and deliver high-quality, person-centred care, assessing risk and developing and reviewing care plans with families and carers.

Qualifications

• Strong communication, assessment and risk management skills essential.
• Ability to work collaboratively within a multidisciplinary team.
• Experience across mental health crisis services or home treatment is advantageous.

Responsibilities

• Lead Professional for young people receiving home treatment, coordinating care.
• Undertake comprehensive mental health assessments and develop clinical formulations.
• Plan, implement and review person-centred care plans in collaboration with families and partners.
• Travel across the service area to deliver community-based care as needed.
