The Digital Health Service provides a mechanism for delivering 21st-century care and connecting people with medical and non-medical sources of support, empowering people to better self-manage their conditions and/or work collaboratively with healthcare professionals to manage their conditions.
The Digital Health Service delivers expert clinical case management using virtual technologies. Patients with shortand long-term conditions may be at risk of deteriorating health, which can result in declining quality of life or avoidable hospital admission.
The Digital Health Team Leader is an essential supervisory role for a service monitoring and responding to markers of declining patient health. Proactive and flexible team management enables team members to deliver the best care possible to patients on the Digital Health Service.
The Digital Health Team Leader is responsible for the general day to day running of EBPC Digital Health team members, Nurses and AHPs working both within the EBPC call centre or remotely.
The expectation is to monitor the work of Nurses, AHPs, Care Navigators, and Clinical Support team members, ensure shifts are covered (by liaising with the Resources Department), and ensure staff can use Docobo and Connected Care correctly. This will involve an element of training and one-to-one assistance to ensure all Digital Health team members are up to date with Docobo and other systems.
EBPC are expanding and recruiting into our Digital health team and looking for a Team Leader to work Full time within our headquarters at Bracknell.
EBPC is a well-respected enabler of high-quality NHS services. Established 27 years ago as an Out of Hours GP service, we have grown significantly and deliver a full range of services, 24/7, to support 111, Primary Care Networks, Digital Health, Integrated Care Systems, Acute and Community NHS Trusts.

GENERAL
This post is one of continual development. The job description is intended as a guide to the principal duties and responsibilities of the post and complements individual objectives set in line with EBPC annual business objectives. Responsibilities will be reviewed periodically in line with service priorities and duties may change or new duties be introduced after consultation with the post holder.
CRIMINAL RECORDS BUREAU DISCLOSURE
This post may require you to hold a current CRB Certificate which EBPC make application for every three years. Under the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1075, applicants for this post are among those who are not entitled to withhold information about any previous criminal conviction.
FLEXIBILITY
The EBPC is currently working in a climate of great change within the healthcare sector. It is therefore expected that all staff will develop flexible working practices within EBPC and across the health network and at other organisation levels as appropriate, to be able to meet the challenges and opportunities of working within the new healthcare environment.
MOBILITY
The post holder will be expected to work at any establishment at any time throughout the duration of their contract, within the location of EBPC as agreed with the post holder.

You also have a personal responsibility and accountability to your team and service to keep yourself up to date and, if you encounter unacceptable practice, to raise it or deal with it through accepted channels.

During the course of your employment you may see, hear or have access to information on matters of a confidential nature relating to the work of EBPC or to health and personal affairs of patients and staff. Under no circumstances should such information be divulged or passed on to any unauthorised person(s) or organisation(s).

If required to do so, obtain, process and/or use information held on a computer or word processor in a fair and lawful way. To hold data only for the specified registered purpose and to use or disclose data only to authorised persons or organisations as instructed.

NO SMOKING POLICY
EBPC have a policy on No Smoking. Staff and visitors are banned from smoking within any of the buildings operated by the EBPC.

During the course of your employment, you are required to inform the company of any changes to your circumstances regarding the outcome of investigations or charges against you as they may affect your employment with the company. This may include Driving License penalties or fines as a result of a driving offence or the occurrence of a crash or accident. Criminal investigations by the police, convictions, investigations by authorities such as HMRC, Fraud offices, Department of Work & Pensions, Social Services.
CHANGE IN PERSONAL CIRCUMSTANCES
During the course of your employment, you are required to notify the personnel department of any changes to your personal circumstances as they occur such as change of address, marital status etc to enable accurate records to be maintained.
