Job summary

Frimley Health NHS Foundation Trust is seeking Associate Practitioners to join the new Interim Care and Support Team (ICAST) to support patients at home before a care package starts. You will contribute to team development, supervise junior staff, and participate in ongoing training.

The role requires at least Level 5 qualification, hands-on care experience, a valid driving license and access to a vehicle. You will work a seven-day roster, with shifts typically 7.5 hours between 8am and 9pm, and

Qualifications

• Foundation degree or NVQ Level 5 or equivalent in a relevant field.
• Experience as HCA/Community Support Worker or Therapy Assistant.
• Experience undertaking patient assessments.

Responsibilities

• Assess patients’ care needs and write care plans using a strengths‑based approach.
• Empower patients to be independent and support networks.
• Deliver hands-on care and maintain high quality of care.
• Work a seven-day rota with shifts 08:00–20:00.
