Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Healthcare Support Worker to join our Integrated Community Rehabilitation Team. You will work with therapists to provide rehabilitation support to adults in their homes and community settings, helping them regain independence and improve wellbeing.

The role involves following care plans, assisting with daily activities, monitoring progress, and contributing to a multidisciplinary team.

Qualifications

• NVQ in Health Care or equivalent or willingness to commit to training.
• Current Driving Licence and access to a vehicle for work purposes.

Responsibilities

• Work under supervision following written care plans and care pathways devised by the Registered Nurse.
• Follow prescribed care plans and treatment protocols.
• Follow HPCT Policies and Procedures.
• Has knowledge and skills to carry out prescribed tasks and will report to Registered Nurse any variances.
• Delegated responsibility for Patient Support Teams administrative tasks.
• Provide general administration support to the team.
