Job summary

RCNi Learning is seeking Associate Practitioners to join the new Interim Care and Support Team (ICAST) to support patients at home before their care package starts. You will work in a community-based role, covering a seven-day roster with shifts 8am–9pm, and you must have access to a vehicle and a valid license.

You will assess patients’ care needs, write care plans, and deliver hands-on care while developing the team.

Qualifications

• Minimum Level 5 qualification in a relevant subject.
• Recent practical, hands-on experience in care.
• Willingness to line manage junior staff.

Responsibilities

• Assess patients’ care needs and write care plans.
• Deliver hands-on care and empower independence.
• Rostered to cover a 7-day service with 8:00–20:00 shifts.
