Gloucestershire Health and Care NHS Foundation Trust in Gloucestershire is seeking a Community Nurse to join the Integrated Community Team. You will deliver nursing care in patients' homes and across community settings, contributing to assessments and care plans.
The role requires experience in health or social care, a UK driving licence, and an NVQ Level 3 in a Health or Social Care subject. We offer a supportive, multi-disciplinary environment with developmental opportunities.
Competenze
- Experience in a health or social care setting.
- Driving licence (UK) and access to own car.
- NVQ level 3 in Health or Social Care or equivalent.
- Care certificate and community experience are desirable.
Mansioni
- Deliver nursing care in community settings as directed by the registered nurse.
- Provide care in patients' homes as part of the Integrated Community Teams.
- Assist with patient assessments and care plan development and documentation.
- Undertake direct patient contact without direct supervision by a nurse.
- Maintain accurate records in paper and computer systems per Trust policy.
- Instigate CPR and other emergency procedures when required.
