Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is hiring a Support Worker to join the Intensive Support Team on the Isle of Wight. This permanent, part-time role involves working with service users to assess, plan, implement and evaluate supportive care in collaboration with the multidisciplinary team.

Your responsibilities include promoting safety, building rapport, assisting with care plans, and supporting service users in personal development and community activities.

• Maintain an up-to-date awareness of developments in Social and Health Care practice.

Responsibilities

• Provide support to an allocated group of service users to promote safety and community living.
• Support service users to identify needs and assist in planning, implementing and evaluating individual care plans.
• Maintain confidentiality in all communications and adhere to standards of care.
• Assist practitioners in administering agreed standards of care to service users.
• Build rapport with service users, including in settings outside the home such as community resources.
• Monitor progress and alert staff to early signs of relapse or changes in mental state.
• Participate in risk assessment and risk management practices as required.
• Attend outpatient appointments and clinical reviews when needed.
• Encourage service users to engage in leisure, social and recreational activities.
