Southern Health NHS Foundation Trust on the Isle of Wight is seeking a motivated Support Worker to join the Intensive Support Team on a permanent, part-time basis (30.75 hours per week).

You will work across boundaries of care and organisations to assess, plan, implement and evaluate personalised care plans, building rapport with service users while supporting practitioners within the Intensive Support Team.

The Community Learning Disabilities Team on the Isle of Wight are seeking applications from highly motivated individuals to work as a Support Worker within the Intensive Support Team. This a permanent, part-time position at 30.75 hours per week.

The post holder will be able to focus on the direct needs of the service users, working across boundaries of care an organisations; develop a rapport based upon companionship and friendship and be able to assist practitioners within the Intensive Support Team to assess, plan, implement and evaluate individual care plans.

At present, the Trust is unable to offer Skilled Worker Visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.
