Qualifications

• Strong communication and collaboration skills with multi-agency partners.
• Ability to gather information, assess risk and decide on appropriate next steps.

Responsibilities

• Respond to Contacts/Referrals from multi-agency partners and the public.
• Gather information, analyse risk and identify levels of need to inform decisions.
• Chair and attend multi-agency meetings to plan support for families.
• Engage families using a strengths-based approach and work with the team to improve outcomes.
