Job summary

Locala Health and Wellbeing is seeking an ITOC Senior Physiotherapist to join our Integrated Transfer of Care team. You will work to ensure safe and timely discharge for patients who are medically fit, coordinating with nurses, OTs, physiotherapists and assistants to tailor care for independence.

The role is 37.5 hours per week across 7 days with 1 in 4 weekends, permanent, with potential development from Band 5 to Band 6 for the right candidate.

Qualifications

• Band 6 development possible for suitable Band 5 candidates.
• Role involves multidisciplinary collaboration in ITOC with emphasis on safe discharge.

Responsibilities

• Support safe and timely hospital discharge for medically fit patients.
• Deliver multidisciplinary reablement and rehabilitation plans.
• Collaborate with social services and Locala community services.
