Job summary

University Hospitals of North Midlands NHS Trust (UHNM) is seeking an Advanced Therapist to join the Integrated Discharge Hub on a fixed-term six-month project. The role supports discharge planning with Band 7 seniority and 37.5 hours per week, with options for full-time, part-time, or job share, including weekend and bank holiday work.

The post welcomes Occupational Therapists and Physiotherapists at Band 7 with discharge planning interest, or Band 6 aiming to develop into a stretch role,

Qualifications

• Degree in Physiotherapy or Occupational Therapy.
• Registered with HCPC.
• Higher qualification related to discharge planning (e.g. masters).

Responsibilities

• Be the single point of contact for the therapy teams for discharge queries and concerns.
• Support the Integrated Discharge Hub with multidisciplinary ward meetings.
• Improve efficiency of therapy flags on CDT to reduce length of stay.
• Review bed list regularly to assess ongoing bed-based service needs.
• Increase patients on pathway 1 with identified and supported therapy goals.
• Provide expert therapy input into the IDH for triage of TOC forms.
• Reduce TOCs with insufficient therapy information for balanced discharge decisions.
• Identify patients who could have gone home with night service.
• Promote positive risk taking in discharge planning and decision making.
