Brent Integrated Care Partnership, hosted by CNWL, seeks a Band 7 Transformation Manager to support Assistant Directors in transforming services across Brent. You will map services, coordinate with voluntary, social care and residents, and plan co-location and integration of Integrated Neighbourhood Teams.

Formación

• Essential criteria include a Masters degree or equivalent professional experience.
• A project management qualification or demonstrable delivery experience is required.
• Experience leading transformation projects within health and/or social care.
• Strong stakeholder engagement and co-production experience is desirable.

Responsabilidades

• Map services to geographical footprints across Brent.
• Identify opportunities for service improvement and integration.
• Develop detailed delivery plans for five neighbourhoods/connect areas.
• Engage stakeholders and communities to support co-production.
