Job summary

The Royal Borough of Windsor and Maidenhead is recruiting two Team Coordinators to support Adult Social Care Hospital Team, Interim Care and Integrated Care services. These full-time, permanent roles offer hybrid working and a salary of £25,706 - £27,306 per year.

You will coordinate referrals, maintain performance trackers and ensure timely, coordinated support while keeping accurate records and communicating with health and social care partners.

Qualifications

• Experience in an administrative, coordination or business support role within a busy environment.
• Excellent organisational skills with the ability to manage competing priorities and deadlines.
• Strong written and verbal communication skills and the ability to work effectively with a range of professionals.

Responsibilities

• Receiving, processing and coordinating referrals across Adult Social Care pathways.
• Maintaining service trackers, waiting lists and performance monitoring systems.
• Updating records on Mosaic and other electronic systems.
• Coordinating communication between hospital teams, community services, care providers and colleagues.
• Providing administrative support to meetings, including reports and agendas.
