The Royal Borough of Windsor and Maidenhead invites applications for a Social Care Practitioner in Maidenhead. This full-time, permanent role offers hybrid working and a salary of £26,895 - £30,132 per annum.

You will join the Integrated care team (LAP) to support residents with multidisciplinary care and enable independent living. Key duties include Care Act assessments, caseload management, and providing signposting and advice.

Qualifications

• Experience of working with people, preferably experience with those who are vulnerable or have disabilities.

Responsibilities

• To manage the allocated workload of residents in their own homes or other care settings, working with families and carers to achieve positive outcomes.
• To undertake assessments and reviews in line with the Care Act 2014.
• To facilitate the implementation of support interventions to meet identified outcomes and promote independence.
