Job summary

The Royal Borough of Windsor & Maidenhead invites applications for two Team Coordinators within Adult Social Care, supporting Intermediate Care, Discharge and Integrated Care pathways. The role focuses on co-ordinating referrals, tracking performance and ensuring smooth system flow.

You will maintain records, improve communication between hospital and community teams, and support multidisciplinary meetings in a fast-paced environment. Hybrid working is available.

Qualifications

  • Experience in an administrative, coordination or business support role within a busy environment.
  • Excellent organisational skills with the ability to manage competing priorities and deadlines.
  • Strong written and verbal communication skills and the ability to work effectively with a range of professionals and partner agencies.
  • Good IT skills, including experience using Microsoft Office applications and electronic record systems.
  • A proactive, flexible and team-focused approach, with excellent attention to detail and commitment to confidentiality.

Responsibilities

  • Receiving, processing and coordinating referrals across Adult Social Care pathways.
  • Maintaining service trackers, waiting lists and performance monitoring systems, escalating risks and delays where required.
  • Updating and maintaining records on Mosaic and other electronic systems to ensure accurate and timely information.
  • Coordinating communication between hospital teams, community services, care providers and Adult Social Care colleagues.
  • Providing administrative support to operational and multidisciplinary meetings, including preparing reports, agendas and documentation.

These are full-time, permanent roles with hybrid working, and a salary of £25,706 – £27,306 per annum.

The role:

As a Team Coordinator, you will provide comprehensive administrative and operational support across Intermediate Care, Discharge and Integrated Care pathways. Reporting to the Service Team Manager, you will play a key role in supporting system flow, coordinating referrals, maintaining performance and pathway tracking systems, and ensuring effective communication between health and social care partners.

Working within a fast-paced environment, you will help ensure residents receive timely and coordinated support whilst maintaining accurate records and supporting service performance.
