Zusammenfassung

Calderdale and Huddersfield NHS Foundation Trust is seeking an Independent Living Officer to support adults in the community who do not require acute hospital admission but need assistance to maintain independence at home.

You will assess needs, arrange equipment, adaptations and assistive technology, and participate in rapid reablement initiatives to reduce hospital admissions and expedite safe discharges.

Qualifikationen

• English and Maths GCSE grade 9-2 (A*-E) or equivalent.
• Competence based level 3 Qualification e.g. NVQ or Diploma in Adult Care.
• Willing towards Trusted assessor Accreditation.

Aufgaben

• Assess a wide variety of situations across Calderdale and provide solutions including equipment, adaptations and assistive technology to support independence.
• Support hospital discharges and contribute to Rapid Reablement Service as part of the Urgent Community Response Team.
• Work with people at home to maximise independence and rehabilitation through reablement and emotional support.
