Job summary

Leeds City Council is hiring a Reablement Support Worker to help adults regain independence after illness or injury. You will work with service users in their own homes as part of the Active Recovery team, delivering short-term, person-centred care and building confidence for daily living.

You will complete the Care Certificate within 12 weeks and pursue a Level 2 Adult Care Worker Apprenticeship within 12 months, with flexible travel across the city and a driving licence required.

Qualifications

• Care Certificate within 12 weeks of starting in post.
• Level 2 Adult Care Worker Apprenticeship within 12 months of starting in post.
• Full training provided.

Responsibilities

• Provide support with personal care and daily routines to enable independence.
• Assist customers to access local amenities and community activities.
• Support with medication per care plans and maintain accurate records.
• Work with the Active Recovery team including therapists.
• Contribute to and follow recovery plans and risk assessments.
