Job summary

SHROPSHIRE COUNCIL is recruiting reablement practitioners to deliver intensive short-term, person-centred care to residents aged 18 and over, with a strengths-based approach to promote independence and wellbeing.

You will support activities of daily living at home—self-care, cooking, washing, dressing, medication administration and mobility—drawing on GCSE English and Maths (C/4+) or an equivalent Level 3 Adult Care apprenticeship with functional skills, plus Level 3 Diploma and Care Certificate.

Qualifications

• GCSE English and Maths at C/4 or higher, or complete Level 3 Apprenticeship in Adult Care including Maths and English functional skills.
• Level 3 Diploma in Adult Care and the Care Certificate.
• Experience and knowledge of domiciliary/homecare with ability to undertake manual handling tasks and strong written/observational/communication skills.

Responsibilities

• Provide intensive short-term, person-centred, strengths-based care to promote independence and wellbeing.
• Assist with activities of daily living in the home: self-care, cooking, washing, dressing, medication administration and mobility.
