Trafford & Stockport College Group (TSCG) is seeking a Transition Coordinator (Inclusion) based at Stockport Campus to support learners with EHCPs through a high-quality, personalised transition into the College. You will work with schools, local authorities and external agencies to ensure effective growth and success.

The role involves building rapport with learners and families, aligning curriculum and support offers with the Preparation for Adulthood agenda, and collaborating with Careers,

Qualifications

• Experience supporting learners with EHCPs and transitions.
• Excellent relationship-building with families, carers and professionals.
• Knowledge of SEND Code of Practice and DfE Prep for Adulthood agenda.

Responsibilities

• Develop rapport with learners and families to support transition.
• Provide curriculum and support advice to enable informed next steps.
• Work with Careers, Guidance and Admissions to tailor support.
