Job summary

Trafford & Stockport College Group (TSCG) is seeking two SEND Transition Coordinators to support young people and families transitioning into college. You will collaborate with schools, local authorities and internal teams to ensure effective, person-centred transitions in line with SEND Code of Practice and the Children and Families Act.

The role requires building rapport with learners, carers and professionals, contributing to Post-16 outcomes and coordinating transition plans.

Qualifications

• Strong written and verbal communication skills.
• Experience supporting learners with SEND in FE settings.
• Ability to help students achieve their goals and stay engaged.
• Empathy, resilience and ability to work with diverse learners.
• Capable of challenging learners appropriately and setting targets.

Responsibilities

• Coordinate transition planning for learners with EHCPs coming into College.
• Work with schools, SENCOs and external agencies to support transitions.
• Develop Transition Plans with aspirations, achievements and support needs.
• Provide guidance on curriculum and support offers to learners and families.
• Assist with enrolment, induction and learner progress reviews.
