Job summary

Chelmsford College invites applications for Inclusion Coach – Transition Lead to support learners with SEND through transitions within and beyond the College. The role drives inclusive, high-quality transition planning across curriculum and support services, with strong collaboration to ensure positive destinations.

The post requires dedication to Preparing for Adulthood, excellent communication and organised programme delivery, plus a valid car for travel between sites. Closing date 27/10/2026.

Qualifications

• Strong commitment to inclusion and improving outcomes for learners with SEND.
• Understands Preparing for Adulthood and enabling independence.
• Experience supporting learners with additional needs in education.
• Highly organised, proactive and able to coordinate multiple transition activities.
• Excellent communication, collaboration and problem-solving skills.
• Ability to inspire confidence and resilience in learners.
• Confident with EHCP processes and multi-agency partnerships.

Responsibilities

• Lead and coordinate transition arrangements for learners with SEND, including entry, progression and exit routes.
• Design and deliver tailored transition programmes for high-needs or vulnerable cohorts.
• Organise bespoke taster sessions and transition activities.
• Track learner progress and transition data to inform interventions.
• Collaborate with SENCO, SEND Team Leader and academic teams to support progression.
• Deliver targeted interventions (1:1 and small groups) to build independence and readiness.
• Support EHCP annual reviews, plans and monitoring outcomes.
• Engage with parents, carers and stakeholders to ensure clear communication.
• Contribute to staff training and promote inclusive practice across the College.
