Job summary

Sue Ryder invites applications for a fixed-term palliative care role in England. The post involves identifying patients nearing end of life, coordinating transfers from hospital to hospice, and supporting advance care planning.

You will lead clinical practice and mentor teams while travelling across the region as required. The role requires NMC or HCPC registration and a full UK driving licence with own transport.

Qualifications

• NMC registered nurse or HCPC registered paramedic qualification.
• Experience meeting the needs of palliative care or long-term condition service users.
• Full UK driving licence and own transport for occasional regional travel.

Responsibilities

• Identify patients nearing end of life or with complex palliative care needs.
• Facilitate timely transfer of suitable patients from hospital to hospice.
• Release hospital capacity for patients needing acute medical care.
• Reduce delays in hospital discharges.
• Improve patient experience in line with patients' wishes.
• Promote completion and appropriate use of Advance Care Plans.
• Strengthen relationships between hospice, acute hospital and community teams.
• Build better working with system partners and neighbourhood teams.
