Job summary

Sue Ryder Thorpe Hall Hospice in Peterborough is recruiting a Clinical In-Reach Practitioner to join our care team. 37.5 hours per week, Monday to Friday, with flexible start/finish times and condensed hours considered. £16.51–£17.98 per hour DOE, plus competitive benefits.

A UK driving licence and own transport are essential for regional travel. You will support early identification of patients nearing end of life, facilitate admissions to hospice, help transfer from hospital and reduce delays

Qualifications

• Registered Nurse or Paramedic with palliative care exposure.
• Clinical leadership and team collaboration skills.
• Ability to travel regionally with own transport and valid licence.

Responsibilities

• Support early identification of patients nearing end of life or with complex needs.
• Facilitate admission of palliative care patients into hospice services.
• Coordinate timely transfer from acute hospital to hospice to free hospital capacity.
• Promote Advance Care Plans and collaborate with hospital and community teams.
