Job summary

Shropshire Council’s START in-house domiciliary care service is seeking a Reablement Practitioner to support adults at home across the county. You will deliver short-term, person-centred care aimed at promoting independence and wellbeing, helping residents regain daily living skills such as self-care, cooking, washing, dressing and medication administration.

Working with people in their own homes, you will apply a strengths-based, asset-based approach and provide tailored support 365 days a year.

Responsibilities

• Assist people to regain and retain skills for daily living (self-care, cooking, washing, dressing, administering medication, mobility) and promote independence.
