Job summary

Alexandra House Support Services Ltd provides care across North Wales and is seeking a Domiciliary Care & Support Worker to deliver person-centred care in clients' own homes and in the community. You will follow individual care plans, support daily routines, and promote independence while maintaining dignity and safety.

You will travel between visits, work a flexible rota including evenings and weekends, and hold a valid UK driving licence with access to a vehicle.

Qualifications

• Experience in care is desirable but not essential.
• Reliable and willing to learn, with good communication.
• Full UK driving licence and access to own vehicle are essential.

Responsibilities

• Provide personal care such as washing, dressing, toileting and hygiene.
• Support daily routines, meals, hydration and household tasks.
• Assist with medication per care plans and training.
• Support mobility, transfers and moving with equipment as needed.
• Support individuals with learning disabilities, autism or complex needs.
• Encourage participation in meaningful activities and independence.
• Assist with appointments and community activities where appropriate.
• Recognise changes in health or wellbeing and respond accordingly.
• Follow care plans and safeguarding procedures; maintain records.
• Communicate with individuals, families and professionals; maintain boundaries.
• Work with colleagues to ensure consistency of care.
