Job summary

Conexus Healthcare CIC seeks three Work and Health Coaches to help people in the Wakefield district return to work or stay in work. Base at Sandal Castle Medical Centre with flexible travel across the Wakefield district.

You will provide personalised support, run work and health assessments, and develop action plans. Training is provided; DBS check is required as part of safeguarding. Hybrid working model and NHS/NEST pension apply.

Qualifications

• IT skills including Microsoft Office and online case management systems.
• Networking with employers, GP practices and community groups.
• Strong time management and organisation skills.
• Knowledge of health and wellbeing support available in Wakefield.
• Knowledge of the local labour market and employment law.
• Understanding of health condition impacts in the workplace.

Responsibilities

• Provide one‑to‑one, personalised support to Wakefield residents.
• Conduct work and health assessments and agree clear action plans.
• Offer career and employment guidance and identify workplace adjustments.
• Refer or signpost to health, wellbeing and community services.
• Manage your own caseload and maintain up‑to‑date records.
