Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a Wellbeing Support Worker Lead to provide drop-in and follow-up support across Berkshire. The role requires travel and operating within a hybrid working model to support clients with mental health needs.

The post will involve developing personalised plans, signposting to housing, financial and social support, and collaborating with CPE teams to promote engagement and recovery across Berkshire.

Responsibilities

• Travel across Berkshire for drop-in support and provide hybrid working to facilitate support.
• Support recovery and self-management for individuals experiencing mental health difficulties.
• Facilitate timely intervention to prevent crisis escalation and potential need for secondary care.
• Develop individual plans and signpost to housing, financial, social and VCSE organisations.
• Maintain strong relationships with statutory and non-statutory agencies within Berkshire under CPE leadership.
