Job summary

Central London Community Healthcare NHS Trust is recruiting passionate Neighbourhood Case Managers for the Hertsmere Neighbourhood Proactive Care Service. This is a part-time role (26.25 hours per week) delivering community-based, person-centred care for residents, focusing on holistic assessments and care coordination.

Qualifications

• Registered nurse or AHP with current professional registration.
• Post-registration education and training relevant to role.
• Non medical Prescriber.
• Experience negotiating and liaising within a multidisciplinary team.
• Experience with complex health needs.
• Experience training students/HCAs.
• Experience in comprehensive assessment using NHS records.
• Experience in care planning and record keeping.
• Experience in NHS community environment.
• Experience in advanced care plans and treatment escalation planning.

Responsibilities

• Oversee care in Proactive Anticipatory Care Service and line manage the Neighbourhood case coordinator.
• Provide proactive, personalised multidisciplinary case management for high-risk patients.
• Facilitate timely multidisciplinary discussion leading to coordinated care plans.
• Undertake comprehensive assessment of complex care needs using electronic records.
