Job summary

Leaders in Care is seeking an experienced Discharge to Assess Social Worker to join a busy local authority in the UK. This hybrid role offers a mix of hospital, community, and home-based working, with support for professional development and wellbeing.

You'll conduct strengths-based assessments, coordinate care after discharge, and ensure services maximise independence while reducing readmissions, working closely with health and social care colleagues.

Qualifications

• Qualified Social Worker with current Social Work England registration.
• Experience in Hospital Discharge, Discharge to Assess, or Adult Community Services.
• Solid understanding of Care Act, Mental Capacity Act, and strengths-based practice.
• Experience using Mosaic or a similar case management system desirable.

Responsibilities

• Completing Care Act assessments within the Discharge to Assess pathway.
• Supporting safe and timely discharges from acute hospital settings.
• Undertaking Mental Capacity Assessments and Best Interest decisions where required.
• Coordinating care packages and community-based support.

Education

A recognised Social Work qualification.
