Qualifications

• Registered social worker with Social Work England.

Responsibilities

• Arrange hospital discharge in coordination with the discharge hub.
• Conduct Care Act assessments, support plans or reviews in the community.
• Collaborate with health, housing and voluntary organisations to promote wellbeing.
