Job summary

Slough Borough Council is seeking a dedicated Hospital Social Worker to join our Hospital Team at Observatory House/Wexham Park Hospital. This permanent, 37-hour role offers hybrid working and a chance to support safe, timely, person-centred discharges within the Discharge to Assess pathway.

You will use strengths-based practice, robust assessments and multi-disciplinary collaboration to promote independence and manage funding streams, with an emphasis on safeguarding and cost-effective care

Qualifications

• Experience in strengths-based social care practice.
• Ability to work in a fast-paced hospital environment and use professional judgement.
• Strong assessment and care planning skills with understanding of funding streams.
• Ability to collaborate within a multi-disciplinary team and understand statutory duties and legal frameworks.
