Job summary

Phoenix Futures Housing Sustainable Tenancy Service (HoSTS) is seeking a Housing Support Worker to assist individuals with complex needs toward stable accommodation and recovery. The role is based on two Essex sites and involves collaborative work with partners and healthcare services to deliver personalised interventions.

You will join a small, dedicated team, manage a busy caseload, and use Outlook/Word to document progress while supporting clients on their journey to independent living.

Qualifications

• Relevant experience in health and social care, ideally with substance use or mental health context.
• Evidence of effective partnership working and networking for client benefit.
• Proficiency in Outlook and Word; familiarity with databases.

Responsibilities

• Provide tailored tenancy sustainment and recovery interventions for HoSTS clients.
• Support individuals to access mental health services, GP referrals, and partner agencies.
• Manage a busy caseload and coordinate with colleagues across two sites.
• Engage with clients to enable move from temporary to stable accommodation.
