Job summary

Momentum Children's Charity is seeking a dedicated Family Support Worker to assist families whose child is in hospital, providing emotional and practical support on paediatric wards and after discharge by phone, text and email.

You will manage a caseload across four days, build strong relationships with hospital staff, and connect families to therapies and external services. DBS check and driving licence are required.

Qualifications

• Experience supporting children, young people and families with sensitive needs.
• Ability to build rapport and listen effectively.
• Understanding of professional boundaries and confidentiality.
• Strong written and verbal communication skills.
• Good time management and organisational abilities.
• Calm, empathetic approach in emotional or sensitive situations.
• Proficiency with Microsoft Office and databases (e.g., Salesforce).

Responsibilities

• Visit families on hospital wards and in intensive care to provide emotional and practical support.
• Keep in touch with families between hospital stays by phone or email.
• Manage your own caseload and prioritise contact by need.
• Attend MDT meetings and share knowledge while keeping records.
• Connect families to therapies, respite breaks and external organisations.
• Support siblings, carers and bereaved families with sensitivity.
• Maintain confidential records and report safeguarding concerns.
• Deliver presentations to hospital professionals with consented anonymised stories.
