Job summary

The Cumberland is seeking a Trusted Assessor to support hospital discharge processes, coordinating access to post-discharge services and ensuring safe, timely transitions for adults.

You will work with Case Managers, Discharge Navigators and a multidisciplinary team to triage and route individuals to appropriate community or in-hospital care, in a role that involves regulated activity with adults.

Qualifications

• DBS check required with Enhanced level including Adults Barred List.
• Involvement in regulated activity with adults.

Responsibilities

• Triages and coordinates access to services following hospital discharge.
• Supports and facilitates safe, timely discharges from hospital into health and social care provision.
• Works with Case Managers, Discharge Navigators and multidisciplinary partners to ensure smooth transitions.
